About this test
Gonorrhea is a bacterial infection that can affect the genitals, the rectum and the throat. CDC recommends a gonorrhea test every year for sexually active women under 25, and for women 25 and older at increased risk. Gay, bisexual and other men who have sex with men should test at least once a year at every site used for sex, and every 3 to 6 months if at increased risk.
The test is a NAAT, which detects the bacteria's genetic material in urine or a swab. CDC names first-catch urine for men and vaginal swabs for women as the best samples, and says patient-collected samples are reasonable alternatives to swabs taken by a provider. A urine test doesn't check the throat or rectum. CDC recommends throat and rectal tests for men who had receptive oral or anal sex.
Gonorrhea is treated with antibiotics. CDC's recommended treatment is an injection, so treatment usually means an in-person visit. Gonorrhea is reportable in every state, and the health department keeps reports confidential.
When to test
CDC doesn't publish an exact window period for gonorrhea. Its guidance after a sexual assault is to repeat STI tests 1 to 2 weeks after exposure if the first test was negative, because an infection may not be detectable at first. Testing 2 weeks or more after sex matches that timing. Test every site that was exposed, including the throat and rectum. If you have symptoms, see a clinician now.
Retesting
CDC recommends a new gonorrhea test 3 months after treatment, whether or not you think your partners were treated. If you had gonorrhea in the throat, CDC recommends a test of cure 7 to 14 days after treatment.
Reported to public health
Positive results for this infection are reported to the state or local health department, which keeps them confidential. That includes tests you order online. What that means for your privacy
Common questions
Can I test for gonorrhea at home?
Yes. In 2023 the FDA authorized the first chlamydia and gonorrhea test that uses a sample collected at home and mailed to a lab. Many home kits test urine or a vaginal swab, and some include throat and rectal swabs. Check the What's included column.
How soon after sex can I test for gonorrhea?
Two weeks after sex matches CDC's timing. CDC's guidance after a sexual assault repeats STI tests 1 to 2 weeks later when the first test is negative. If you have symptoms, see a clinician now.
Do I need a throat or rectal swab?
If you had oral or anal sex, yes. A urine test doesn't check those sites. CDC recommends throat and rectal tests for men who had receptive oral or anal sex, and says about 70% of gonorrhea and chlamydia infections in men who have sex with men might be missed with urine-only testing.
How is gonorrhea treated?
With antibiotics. CDC's recommended treatment is an injection, so you'll usually need a visit to a clinic, urgent care or your doctor. Many testing providers can refer you or arrange a consult for a positive result.
Is gonorrhea reported to the health department?
Yes. Gonorrhea is reportable in every state, including for tests ordered online. CDC says these reports are kept confidential. The health department may contact you to check on treatment and to offer help notifying partners.
